Lead Base Paint Addendum (4 pages Part A; Part B; Part C; Part D.  ) must be attached to the offer if the home was built prior to 1978.

Owner Occupied Certification Form must be included with the offer if the purchaser is going to occupy the property.

The Forfeiture and Extension Addendum must be signed and attached to all offers. PLEASE NOTE THAT THIS IS A REVISED FORM (rev. 10/18/99) AND YOU MUST USE THE NEW FORM.

Original Revised Letter of Commitment (on Lender's Letterhead complete with lenders address and phone number or on the Broker's letterhead for cash deals) is required with all offers.

A Hold Harmless Agreement and a Utility Authorization form must be delivered to our Irvine office closing department prior to utilities being activated. In order to activate utilities, the buyer must first have a signed contract and the property must be in escrow.   This form will be signed for approval and faxed back.  You may use this form to authorize the appropriate companies to activate the utilities.  All expenses (including fees, deposits or damage that may occur as a result of the activation) are the responsibility of the buyer.

Bid Cancellation Notice. You can either (1) e-mail Golden Feather Realty with the Bid Confirmation Number, broker's and buyer's name, case number and address of the property, or (2) Complete the Bid Cancellation Notice and fax it to us. This form is only valid if sent prior to the bid opening and is not valid after that time.

Extension Request. If a sale is not going to close within its designated time period - for any reason - an Extension Request must be submitted in a timely manner so that Golden Feather receives the request before the expiration of the contract. Extension Requests received after contract expiration are subject to being returned without being considered.

All Extension Requests must be submitted through HUD's designated Closing Agent with extension fees paid up front. Do not send your Extension Request to Golden Feather Realty directly. Fees are based on the contract sales price: (a) $10 per day when the sales price is $25,000 or less, (b) $15 per day when the sales price is more than $25,000 and no more than $50,000, and (c) $25 per day when the sales price is over $50,000.

Extension Requests must be made on the proper form, stating the reason(s) for the delay, and must be accompanied by the necessary documentation. The documentation submitted with the request must establish the cause of the delay and that mortgage approval is imminent during the extension period.

NON-PROFITS:  A Land Use Restriction form must be attached if the property is eligible for a 30% discount.

OFFICERS AND TEACHERS must submit a completed Certification form for all Officer/Teacher Next Door purchases.
